Last week, in my first post in this two-part series on Sonoma County blepharoplasty (eyelid surgery), I outlined goals and basic options for eyelid surgery and explained why the choice of a surgeon can make such a difference to the accuracy of results for this type of surgery. This week, I will highlight some of the most popular and effective approaches I use to perform blepharoplasty at my Sonoma County cosmetic surgery offices, including both “straightforward” blepharoplasty and approaches that combine eyelid surgery with another procedure.

Option 1: Blepharoplasty Alone

For younger patients blepharoplasty alone may be sufficient to “open up” the look of the upper face by smoothing out loose tissue either above or below the eye. Lower-eyelid surgery offers a number of options, for example to adjust the appearance of the “tear trough” below the eye, specifically. For upper-eyelid blepharoplasty, the surgeon may need to correct the position of loose lid muscles at the same time as he or she adjusts the loose skin or fatty tissues.

Some older patients will see sufficient improvement with blepharoplasty alone, though in general their other facial tissues will also show significant signs of wear that may need to be addressed at the same time.

This option may be right for you if:

  • Your eyelid sagginess results mainly from hereditary traits rather than aging
  • Your skin quality is good and you are not experiencing looseness or noticeable wrinkles in other areas of your face

Option 2: Blepharoplasty and Facelift or Brow Lift

For older patients especially, many of the same factors (sun damage, gravity, changing tissue quality) that cause eyelid tissues to sag will also affect the rest of the face. Because the cheeks and the brows “frame” the eyes, patients who are experiencing noticeable sagginess in their cheeks or brows will often combine their Santa Rosa blepharoplasty with a facelift or brow lift (or both) to correct these other areas and create balanced, more natural-looking results.

This option might be right for you if:

  • Low brows contribute to your eyelid sagginess
  • “Tear troughs” and upper-cheek wrinkles merge at the corners of your eyes, and/or vertical facial lines around the mouth give your face a saggy appearance

Option 3: Blepharoplasty and Skin Rejuvenation

Sun damage is a common problem. Many of my younger Sonoma County facial cosmetic surgery patients have relatively good skin, but they are starting to see early signs of facial aging because of sun exposure. For patients like these, laser resurfacing or a medium-intensity (TCA) chemical peel can improve skin quality and tone, while stimulating the skin to maintain a more youthful appearance. Both of these procedures also have some tightening effects, which can minimize the appearance of moderate facial folds.

This option might be right for you if:

  • Your skin is not showing sagginess, deep hollows, or severe facial folds
  • Sun damage has given your skin a papery look or “age spots,” and you would like to improve its texture and tone
